Any member of the LTF is eligible to join sick back within 60 days of receiving tenure.  You will be sent a form from Nancy DeMarco which must be signed and sent back to her within 60 days.  Once your membership into the sick bank is complete, you will donate 3 of your sick days into the bank. 

A complete list of all sick bank rules is available in every building.
